My Life My Future 2.0 - Cohort Registration Form
Welcome
Thanks for your interest in this project. This form should take 5-10 minutes to complete.
YourStory, in collaboration with ClearView Research, are recruiting young people in Year 7 (aged 11/12) and Year 10 (aged 14/15) who live in London and are impacted by long-term health conditions to take part in the My Life My Future programme.
My Life My Future is a 7-year programme that combines personalised 1-1 mentoring, peer group activities, family/carer support and ongoing research to understand and improve how health and education systems support young people over time.
With this programme, we want to create better experiences and outcomes for young people impacted by long-term health conditions.
When we say "young people impacted by long-term health conditions", this includes young people living with a long-term physical or mental health condition (such as asthma, type 1 diabetes, sickle cell disease, epilepsy, IBD, anxiety/depression, ADHD, or autism), as well as young people impacted through a family member or someone they care for who has a long-term health condition.
The young people selected for the programme will take part in:
1-2-1 personal mentoring: online or in-person sessions, starting weekly and then gradually phasing out to bi-weekly, then monthly, then based on need
Peer-group meet-ups: group sessions every six months to meet peers, share experiences, stay connected and build a supportive community
Family and carer support: optional access to a Family Practitioner to offer extra support to families and carers where beneficial
Research activities: simple six-monthly data collection activities to capture experiences across school, college, work, and healthcare.
The programme is scheduled to start in early 2027. Before it starts, those selected to take part will receive more information about the programme, take part in an onboarding conversation and fill out an introductory survey.
This is a unique opportunity for young people to gain a dedicated mentor, build confidence, meet peers who understand their journey and make their voice heard, as well as shape future support and services through direct feedback!
The deadline to register is Friday 13th November 2026, 11:59pm. If you're picked, we'll let you know by Friday 20th November 2026 and send you more details. Please note that spaces are limited, and we'll select participants on a first come first served basis and based on achieving a diversity of people in the programme.
IF YOU’RE A YOUNG PERSON INTERESTED IN THIS, WE WILL NEED YOUR PARENT/GUARDIAN TO COMPLETE THIS FORM.
